As part of delivering integrated health and social care services on the Isle of Wight we have established a number of joint teams comprising of staff from multiple organisations. Currently the teams still rely a lot on paper and are co-located rather than working in a truly integrated way.
We would like to understand the interactions between services and develop a solution for sharing client level data on a common platform such as sharepoint. The desire is to lever existing tools whilst ensuring greater joint working and maintaining security of data.
